Description An account of the resource Resumen: Las pruebas de diagnóstico informan el estado de salud del paciente; pueden llevar a valoraciones clínicas y decisiones terapéuticas incorrectas, notándose la importancia de evaluar la precisión diagnóstica de una prueba. Con el propósito de poner a consideración de la comunidad científica la Prueba Diagnóstica (PD) Razón A:B, analizar su validez como alternativa al ángulo ANB, prueba gold estándar, en determinación de relación sagital intermaxilar de un paciente, que permita la toma de decisiones clínicas terapéuticas, se realizó una investigación radiográfica cefalométrica, tipo transversal, observacional, diseño descriptivo, analítico, correlacional; se realizó análisis test-retest, análisis de asociación, correlación, sensibilidad y especificidad, valores predictivos positivos y negativos, razón de verosimilitud positiva y negativa, de prueba diagnóstica Razón A:B, tomando como gold estándar el ángulo ANB. Según los resultados, a través de curvas ROC, se determinaron valores de corte para Razón A:B: Clase I: 0,77 hasta 1,4; Clase II: <0,77; Clase III: > 1,4. Razón A:B presenta alta correlación, negativa e inversa con Áng. ANB con Correlación no Paramétrica rho de Spearman; tipo facial no afecta diagnóstico de relación intermaxilar de PD Razón A:B; Razón A:B, presenta capacidad moderada como prueba diagnóstica de la Clase I; presenta validez diagnóstica suficiente para diferenciar relación esqueletal de Clase II y Clase III, usando como gold estándar el ángulo ANB. En conclusión, la Razón A:B, medida proporcional geométrica, presenta validez diagnóstica para diferenciar la Clase I, Clase II y Clase III esqueletal, no está influenciada por punto anatómico Nasión ni por el tipo facial. Abstract: Diagnostic tests report the patient’s health status; they can lead to incorrect clinical assessments and therapeutic decisions; noting the importance of evaluating the diagnostic accuracy of a test. In order to put the Diagnostic Test (DT) Reason A:B for the consideration of the scientific community, analyze its validity as an alternative to the ANB angle, standard gold test, in determining the intermaxillary sagittal relationship of a patient, that allows therapeutic clinical decision making. A cephalometric radiographic investigation was carried out, crosssectional, observational, descriptive, analytical, correlational design; test-retest analysis, association analysis, correlation, sensitivity and specificity, positive and negative predictive values, positive and negative likelihood ratio, diagnostic test Reason A:B were performed, taking ANB angle as standard gold. Results: through ROC curves, cut-off values were determined for Reason A:B. Class I: 0.77 to 1.4, Class II: up to <0.77; Class III:> 1.4. Reason A: B has a high, negative and inverse correlation with Ang. ANB with Spearman rho Nonparametric Correlation; facial type does not affect diagnosis of intermaxillary relationship with Reason A:B, but if it is affected through ANB angle test; Reason A:B, presents moderate capacity as a diagnostic test for Class I; presents sufficient diagnostic validity to differentiate Class II and Class III skeletal relationship, using the ANB angle as standard gold. In conclusion, Reason A:B, geometric proportional measure, presents diagnostic validity to differentiate Class I, Class II and Class III skeletal, is not influenced by anatomic point Nasión or facial type.
