Description An account of the resource Resumen: La anomalía de Kimmerle o Ponticulus posticus, es una variante anatómica de la vértebra cervical C1 (Atlas), se produce cuando la membrana atlanto-occipital posterior se osifica de forma completa o parcial con relación al paso de la arteria vertebral, siendo la mayor parte de las veces un hallazgo radiográfico. La sintomatología puede o no estar presente, manifestándose como cefalea, vértigo, dolor ocular, fotofobia, cervicalgia e isquemia vertebro-basilar. Estudios realizados en Chile han encontrado una prevalencia cercana al 38%. Un paciente de sexo femenino de 19 años y 5 meses, con antecedentes médicos de cefalea, acudió al Hospital Peñaflor de la Ciudad de Santiago de Chile en febrero de 2025 para la toma de una tomografía computarizada de haz cónico (CBCT) de cráneo completo y telerradiografía, para un estudio de ortodoncia previo al tratamiento. Durante el procedimiento, se pudo observar en la proyección sagital la presencia del Ponticulus posticus en la vértebra C1. Se realizó la anamnesis respecto a su sintomatología, en la cual relató migrañas recurrentes, que sólo cesaban con el uso de analgésicos y descanso a oscuras. También refirió en más de una ocasión haber sufrido tensión muscular y pre-síncope (visión borrosa, temblor y debilidad). Los odontólogos cuentan con una herramienta valiosa como la imagenología craneofacial, la cual permite detectar la anomalía de Kimmerle y establecer un diagnóstico diferencial con otras alteraciones del territorio maxilofacial. Reconocer esta condición de manera oportuna es fundamental para informar al paciente y evitar conductas de riesgo en su vida diaria, así como para contribuir al manejo adecuado de la sintomatología asociada. Es importante considerar que la cirugía no constituye la primera opción terapéutica en estos casos. Abstract: Kimmerle anomaly, or Ponticulus posticus, is an anatomical variant of the C1 cervical vertebra (Atlas). It occurs when the posterior atlanto-occipital membrane ossifies completely or partially in relation to the passage of the vertebral artery. It is most often a radiographic finding. Symptoms may or may not be present, manifesting as headache, vertigo, eye pain, photophobia, neck pain, and vertebrobasilar ischemia. Studies conducted in our country have found a prevalence of approximately 38%. A 19-yearand- 5-month-old female patient, with a medical history of headaches, presented at Peñaflor Hospital in Santiago, Chile, in February 2025 for a conebeam computed tomography (CBCT) scan of the full skull and a lateral cephalometric radiograph, as part of a pre-orthodontic treatment study. During the procedure, the sagittal projection revealed the presence of a Ponticulus posticus on the C1 vertebra. A medical history interview was conducted regarding her symptoms, during which she reported recurrent migraines that only subsided with the use of analgesics and resting in a dark environment. She also reported having experienced muscle tension and pre-syncope on more than one occasion (including blurred vision, tremors, and weakness). Dentists have a valuable tool in craniofacial imaging, which allows for the detection of Kimmerle’s anomaly and the establishment of a differential diagnosis with other conditions in the maxillofacial region. Timely recognition of this condition is essential to inform the patient and help prevent risk-related behaviors in daily life, as well as to contribute to the proper management of associated symptoms.
