Description An account of the resource Resumen: El propósito de esta publicación es presentar la resolución ortógrada de dos casos clínicos inusuales en los que perforaciones radiculares fueron tratadas erróneamente como conductos radiculares, con obturación endodóntica directa en el espacio periodontal. Las piezas dentarias tratadas correspondieron a dos pacientes derivadas a la Cátedra de Endodoncia de la Facultad de Odontología de la Universidad de Buenos Aires. En ambos casos, la inspección radiográfica y clínica reveló la presencia de gutapercha en perforaciones radiculares, sin el tratamiento del conducto real. Los procedimientos realizados incluyeron remoción ortógrada del material obturador mediante tracción pasiva con micropinzas bajo magnificación, localización y tratamiento del conducto original, y sellado del defecto con el material más adecuado, según la localización de la perforación. Ambos tratamientos fueron completados en una sola sesión. Ambos casos destacan la importancia de la correcta identificación del sistema de conductos, el uso de magnificación y la selección adecuada de materiales. Abstract: The purpose of this publication is to present the orthograde resolution of two unusual clinical cases in which root perforations were mistakenly treated as root canals, resulting in direct endodontic obturation into the periodontal space. The treated teeth belonged to two patients referred to the Endodontics Department at the School of Dentistry, University of Buenos Aires. In both cases, clinical and radiographic examination revealed the presence of gutta-percha within root perforations, with no treatment of the actual root canal. The procedures performed included orthograde removal of the filling material using passive traction with micro tweezers under magnification, localization and treatment of the original root canal, and sealing of the defect with the most appropriate material, according to the location of the perforation. Both treatments were completed in a single session. These cases highlight the importance of correct identification of the root canal system, the use of magnification, and proper material selection.
