Description An account of the resource Resumen: Los procedimientos endodónticos regenerativos (REPs) determinaron un cambio de paradigma en el tratamiento de dientes permanentes inmaduros con diagnóstico de necrosis pulpar y ápice abierto mayor a 1,1 mm. Se reporta el caso clínico de un paciente de 14 años de edad, que acudió con una fístula de 2 años de evolución, por necrosis en la pieza 3.5. En el análisis radiográfico se visualizaba periodontitis apical y ápice incompletamente desarrollado. Se aplicó el protocolo de los REPs, que permitió no sólo la resolución de la periodontitis apical, sino también el cierre apical junto con el crecimiento en longitud y ancho de las paredes dentinarias radiculares; alcanzando la pieza dentaria 3.5 los objetivos propuestos por la Asociación Americana de Endodoncia (AAE). Abstract: Regenerative endodontic procedures (REPs) determined a paradigm shift in the treatment of immature permanent teeth with a diagnosis of pulpal necrosis and open apex greater than 1.1 mm. The clinical case of a 14-year-old patient is reported, who came with a fistula of 2 years of evolution, due to necrosis in piece 3.5. Radiographic analysis revealed apical periodontitis and an incompletely developed apex. The REPs protocol was applied, which allowed not only the resolution of the apical periodontitis, but also the apical closure together with the increase in length and width of the root dentin walls; reaching the dental piece 3.5 the objectives proposed by the American Association of Endodontists (AAE).
