<text>Abstract: Hypochlorous acid (HOCl) is the bactericidal fraction of sodium hypochlorite (NaOCl) and is generated during its dissociation in water; moreover, it is an endogenous molecule that is part of the so-called reactive oxygen species. Sodium hypochlorite (NaOCl) is currently the most commonly used irrigant in endodontic treatments, as it shows acceptable clinical and radiographic results; however, its high toxicity should not be overlooked, especially in young patients where underdeveloped apices have greater permeability to periapical tissues necessary for regeneration therapies. The objective was to evaluate the response of periapical tissues to irrigation of the canal with HOCl in an experimental model in rats. Four-week-old Wistar rats were anesthetized with ketamine/xylazine for an occlusal opening of the lower first molars using a carbide bur. The dental pulp was disrupted, and it was irrigated with saline solution (S) in the left molar and 2.5% NaOCl or 0.05% HOCl in the right molar. The control group (C) received no treatment. The animals were euthanized at 2 or 7 days, and the mandibles were dissected for analysis via microcomputed tomography (microCT) and histology (H&E). MicroCT showed that at 2 days, the interradicular bone volume (IBV) tended to be lower in S and NaOCl compared to HOCl (S: 1.28±0.46; NaOCl: 1.24±0.38; HOCl: 1.52±0.29), with no significant differences (NS). At 7 days post-treatment, the IBV showed a trend towards decrease in S compared to the other studied conditions (S: 1.97±0.53; NaOCl: 1.42±0.44; HOCl: 2.11±0.37), NS. The periodontal ligament space was smaller in S compared to HOCl at 2 and 7 days (NaOCl: 176±35; HOCl: 183±5). Histologically, the S group showed exacerbated inflammatory processes on the second day due to the presence of extensive lymphomonocytic infiltrate in the canal, coronal pulp, periodontal ligament, and medullary spaces of the IBV, extending up to one-third of the total height of the molar, in contrast to C, which showed no infiltrate. In NaOCl and HOCl, inflammatory infiltration was limited to the periapex and the lower third of the intraradicular bone marrow. Based on these preliminary results, we could conclude that the response of periapical tissue to irrigation with HOCl is biologically acceptable and similar to the action of hypochlorite.</text>
